There has been a change to the process for students to self-certify. The Student Health Certification Form will be made available for completion directly within PowerSchool. A step-by-step guide of that process for both desktops/laptops and mobile devices is attached.
The form must be completed each school day for all students, even if they are not scheduled for in-person instruction. The time by which the form must be completed is embedded within the instructions. The form must be completed prior to students entering the building if they are scheduled for in-person instruction.
Parents are expected to begin completing the Student Health Certification form within PowerSchool effective Monday, March 8th. Students will no longer receive the Google Form from Mr. Willie Stevens in the morning. The process itself is significantly shorter and if no symptoms are present, it just requires clicking ‘I confirm’ and then submitting the form. This eliminates the potential for typos or incorrect information being entered as there are no actual text entry boxes. The new Student Health Certification Form will also be used to track attendance for asynchronous Wednesdays.
